The Adapted Physical Activity Program (APAP) is an evidence-based physical activity promotion service for people with chronic and complex physical and mental health conditions.

Our home and community-based service aims to empower clients to improve their functional outcomes, mental health and/or social re-entry by giving them the skills, knowledge and confidence needed to commence and maintain a physically active lifestyle.
APAP is for people who would like to improve their health and activity levels through a fun and supportive program. We don't build everything we do around your health condition or disability. Instead, we help you to explore your potential and work out ways to optimise your current health and prevent sickness and poor health in the future.
Most people know physical activity is good for you, but that doesn’t make it any easier to be more active. We understand the daily demands of life, the impacts of illness and or disability can make it hard to be active. We work with you to navigate these barriers and find activity that suits you and your lifestyle.
Our APAP officers are accredited exercise physiologists who specialise in an evidence-based holistic approach to physical activity promotion, incorporating behaviour change strategies, value identification, motivational interviewing, trauma-informed care to assist you in identifying and incorporating physical activities that are safe, effective, enjoyable, meaningful and sustainable.
We put considerable time and energy into identifying meaningful activities that you enjoy so you will want to stay active (rather than just giving you exercise programs that we think are good but which you don't enjoy and may never do).
We provide services in the greater Brisbane, Redlands, Ipswich, Caboolture/Bribie and Gold Coast regions
What is included in the program:
- face-to-face sessions delivered by an Accredited Exercise Physiologist in a home or surrounding community (we come to you!)
- a comprehensive initial assessment of the participant’s physical capacities, social support, weekly routine and home/community environment
- individualised exercise program and/or physical activity routine
- all support materials (e.g. program cards, instruction videos, activity diaries, reminder notices etc)
- follow-up phone calls/e-mail/text and other between-session support
- Development of specific skills and knowledge that will help participants get active and stay active including goal-setting, monitoring progress, time management and building social support
- assistance finding and accessing community services (e.g., finding programs, strategies for pool access, joining sports clubs)
- Liaising with participant's health providers to ensure a collaborative approach.
Eligibility
Our focus is Individuals with complex physical, neurological and mental health conditions, from children through to older adults. Specific conditions include, but are not limited to:
- Post-traumatic stress disorder (PTSD)
- Mental Health Conditions
- Spinal Cord Injury
- Brain Injury
- Amputations

Funding Options
Many individuals who wish to participate in APAP may be eligible to receive funding to support their participation in the program.
NDIS
Self-managed and Plan-managed NDIS participants who have available funding in their NDIS Capacity Building supports Budget in the areas of ‘Improved Health and Wellbeing’ and or ‘Improved Daily Living’.
Medicare - Chronic Disease Management Plan
Your GP may refer you to APAP through a chronic disease management(CDM) referral as part of your allied health care.
Private Health Insurance
If you have private health insurance you may be eligible for a rebate under your cover for exercise physiology. Please check your eligibility and rebate amount with your private health insurance provider.

Other pricing information
All programs will include an additional travel cost from the University of Queensland, St Lucia to the client at 0.85c/km. An additional fee will be charged for all travel times greater than 45 minutes from the University of Queensland, St Lucia.
